Please keep in mind we have a two day/ week minimum requirement. We will confirm what days are available & start date. If you aren't sure or are flexible, just let us know and we can help you determine the right schedule.

Billing

Payment Policy: Payment due at time of service, for the month. Will be billed monthly via Pet Check of the 1st of each month. We will provide you with your Pet Check Account log in info. Cancellation Policy: no cancellations unless due to illness. Make up day(s) will be allowed pending availability. For more info refer to the polices form. More detailed info will be emailed to you.

Liability Waiver & Policies

1. Bark Avenue will endeavor to offer only sound, safe, and responsible care for my dog(s). However, I have been told and understand the risks inherent in dog day care, including but not limited to interactions with other dogs and potential exposure to disease and parasites such as but not limited to fleas. Further, I am and will remain responsible for the actions of my dog at all times and I hereby agree to indemnify and hold harmless Bark Avenue of any and all claims of injury, expense, costs, or damages caused by the actions of my dog while under Bark Avenue care. I have been told by Bark Avenue and understand the inherent risks of owning a dog, including but not limited to the risk of dog bites to myself or others. I recognize that Bark Avenue is not responsible for any unintentional errors, omissions, or incorrect assertions. I understand that the recommendation of any other product or service is not a guarantee of my satisfaction with that product or service.

Reduction of COVID-19 Exposure*

I warrant and represent that I will remain aware of and abide by local law and public guidance on reducing COVID-19 exposure, and will follow best practices in relation to Bark Avenue Doggy Daycare’s services regarding social distancing and sanitation, including the wearing of a mask, maintaining six feet of distance. You may not bring your dog if you have fever or a cough. This includes anyone in same household with symptoms or confirmed positive COVID-19 test as well as anyone you've had contact with who has a confirmed positive COVID-19 test. I understand that engaging Bark Avenue Doggy Daycare’s services carries a risk of COVID-19 exposure and I agree to indemnify, release, and hold harmless Bark Avenue Doggy Daycare of any and all claims of exposure or contraction, whether made by myself or others, to the extent permitted by applicable law For dog daycare: I understand that I may be regularly asked to confirm my health and the health of all other members of my household, including awareness of any fevers or other symptoms. I represent and warrant that I will provide full and complete, accurate information as known to me. I shall notify Bark Avenue Doggy Daycare immediately upon coming down with COVID-19 symptoms, or having been in contact with anyone testing positive for the virus.

In The Event of An Emergency*

2. SELECT YES OR NO: I authorize emergency medical care to be provided by the above-named veterinarian, or an appropriate alternate to be determined by Bark Avenue in the event my regular veterinarian is not available or that closer care is required. I will reimburse Bark Avenue for any charges related to emergency care. YES: I authorize Bark Avenue to administer or seek first aid and resuscitative care as determined appropriate by Bark Avenue and I agree to indemnify and hold harmless Bark Avenue for all and any results thereof. NO: I DO NOT authorize Bark Avenue to administer or seek first aid and resuscitative care as determined appropriate by Bark Avenue and I agree to indemnify and hold harmless Bark Avenue for all and any results thereof.
